Version 0.21.1 released

The 0.21.0 win32/dos releases came with new binutils, causing several problems. This was reverted, and now the 0.21.1 win32/dos versions include the same binutils as FreeBASIC 0.20.0.

Also, the FB manual was recompiled to fix a missing-content issue with the CHM's index.

Besides that, there have been some smaller fixes regarding the FB compiler/libraries, you can find them listed in the changelog.
